Subject: Block-by-block Instructions for Evaluation Process

The following are detailed instructions for completing each block on the UC Berkeley Midshipman Evaluation form. Individuals and their evaluators must complete the following blocks:

 

Midshipman:             Blocks 1-12, 14

First Round:              Blocks 13, 15-18, 20-23

Second Round:          Blocks 15-18, 20-22, 24

 

1.  Name:  Use ALL CAPS. Last name is followed by a comma, all other names are separated by spaces only. Omit spaces and punctuation within a name. If you have no middle initial, put “NMN” after first name. Examples:

            SMITH, JOHN M

            JONES, ANDREA NMI

            THISTLEBURRY, WINTHORPE K JR

            STJOHN, MELVIN J II

 

2.  Class year: Use ALL CAPS. Grade or rank which you are actually wearing on the date listed in block 5. Examples:

            MIDN 1/C

            MIDN 4/C

            OC

            SGT

           

3.  Company/Platoon/Squad: Companies are listed by letter; platoon and squad by number. Each is separated by a slash. Department heads list which squad they are in only. If you are staff, put STAFF followed by your billet. Examples:

            A/1/2     

            B/2/1     

            STAFF/A/2 PLT LDR

            STAFF/BXO 

 

4.  Semester:  Self explanatory. For those on quarter systems, put “Fall” for fall quarter, “Spring” for winter or spring quarter.

 

5.  Date of report:  Enter the date the completed evals are due to the BCO. This should be put out in the letter of instruction for the eval process. Use YYMMMDD format, ALL CAPS. Examples:  04MAY15, 03DEC07

 

6.  Type of Report:  For Final Evals, put an X in “Final.” For all other reports, put an X in “Special.” Use ALL CAPS.

 

7a.  PRT:  List the data for your most recent official PRT. Scores are abbreviated as follows: OUT, EX, GOOD, SAT; and HI, MED, LOW, MAR and FAIL, separated by a dash. Dates are in the same format: YYMMMDD. Use ALL CAPS. Examples:

            OUT-HI

            EX-MED

            GOOD-LOW

            SAT-MAR

            FAIL

 

7b.  Height/Weight/Body Fat:  Mark within standards (WS) or not within standards (NS). You will be told if you are not within standards. Use ALL CAPS.

 

8.  Sail Qual:  If you have completed sail quals, put your rating here. If not, put none. Use ALL CAPS.

 

9.  Swim Qual:  If you have completed swim qual, put your rating here and the date you took the test (YYMMMDD). If not, put none. Use ALL CAPS.

 

10a.  Billet Assigned:  List your primary billet here. Use ALL CAPS. If you are just a squad body, list “SQUAD BODY” and your assigned department separated by a slash. Primary billets include:

            SQUAD BODY/N3

SQUAD LEADER

N3 DEPARTMENT HEAD

B/2 PLT LDR

A CO CDR

 

10b.  Major:  List your declared OR intended major here, followed by the degree. Use ALL CAPS. If you are a double major, list them both separated by a slash. Major and degree are separated by a comma. There are no periods in the degree. Examples:

            POLITICAL SCIENCE, BA

            ANTHROPOLOGY, BA/HISTORY, BA

            MECHANICAL ENGINEERING, BS

 

10c.  Collateral Duties:  List the details about your NROTC duties here. Use ALL CAPS. These include specific duties of your billet and voluntary activities. List as many activities as you have participated in. Examples:

            ADP MEMBER

            ADP PLATOON SERGEANT

            ACTING DEPARTMENT HEAD

            UC DAVIS SUPPLY REP

            COLOR GUARD MEMBER

            VA HOSPITAL VISIT

            FLEET WEEK TOUR

 

11.  Honors/Awards:  List your awards, scholarships, and other prizes from NROTC and the greater community here, separated by commas. Use ALL CAPS. These include awards given by the unit (ribbons) and your university. Include all awards from the current and last semester. Examples:

            ACADEMIC ACHIEVEMENT

            MILITARY APTITUDE

            CHRISTIE NROTC ALUMNI SCHOLARSHIP

            REGENTS SCHOLAR

           

12.  Date Completed:  Date you passed your eval to your first round evaluator. Use ALL CAPS, YYMMMDD format.

 

13.  Signature of Counselor:  This is signed by the first round evaluator. If signing in pen, use your normal signature. If signing electronically, use S/MIDN and your last name in ALL CAPS. Example: S/MIDN JONES

           

14.  Signature of Counseled:  This is signed by the person whose name appears in block 1. If signing in pen, use your normal signature. If signing electronically, use S/MIDN and your last name in ALL CAPS. Example: S/MIDN JONES

 

15-20.  Performance Traits:  See the document “FINAL EVAL GUIDELINES” for instructions about grading criteria for blocks 15-20. Use ALL CAPS for the X’s. For Midterm evals, first round evaluators use the top row of brackets designated “midterm.” For final evals, evaluators use the second row (groups of three) brackets. First round evaluators use the leftmost bracket of the triad, second round the center, and class advisors the rightmost bracket. If you genuinely do not have enough experience to evaluate the individual, check NOB in the left column. MIDN EVALUATORS ALWAYS CHECK “NOB” FOR BLOCK 19.

 

 

 


In this example of a final eval, the first round evaluator has marked a 4, the second round a “NOB,” and the class advisor a 3.

 

21.  Growth Potential:  Record the average of scores in blocks 15-20. A 1 corresponds to “Below Standards,” a 5 corresponds to “Exceeds Standards,” et cetera. Round as needed (e.g., a 3.5 rounds up to 4, which is “Above Standards”). Use ALL CAPS for the X’s.

 

22.  Billet Recommendation:  Evaluators can make a recommendation for the person’s next billet on the appropriate line. Use ALL CAPS.

 

23.  Comments on Performance:  The first round evaluator uses this space to make their comments on the individual’s performance. All 1’s and 5’s given in blocks 15-20 must be explained here with a bulleted notation. Comments should cover both performance over the semester and suggestions for future improvement. Specific examples help. Use normal sentence case. At the end, sign your comments electronically as explained for block 13.

 

24.  Comments on Performance:  The second round evaluator uses this space to make their comments on the individual’s performance. All 1’s and 5’s given in blocks 15-20 must be explained here with a bulleted notation. Comments should cover both performance over the semester and suggestions for future improvement. Specific examples help. Use normal sentence case. At the end, sign your comments electronically as explained for block 13.
